Tones Color Lab – Dallas, Texas

By signing below and or checking the box during check-out/booking service, you acknowledge that you have read, understood, and agreed to the following terms and conditions prior to receiving services at Tones Color Lab.

Consultation Acknowledgment

I confirm that I have participated in a consultation and have honestly disclosed my complete hair history, including but not limited to:

​

  • Previous color treatments

  • Chemical services (relaxers, perms, keratin, bleach, etc.)

  • Use of box dyes or at-home treatments

  • Current hair condition

 

I understand that failure to disclose accurate information may affect results and increase the risk of damage.

Chemical + Color Services Consent 

I understand that chemical services (including color, lightening, toning, relaxers, and keratin treatments) involve the use of professional products that may cause:

​

  • Hair dryness or breakage

  • Scalp irritation or sensitivity

  • Unexpected or undesired color results

 

I acknowledge that:

  • Results are not guaranteed

  • Multiple sessions may be required to achieve desired results

  • My hair condition may limit achievable outcomes

 

I voluntarily consent to proceed with these services.

Allergy & Sensitivity Waiver

I understand that allergic reactions can occur with hair products.

​

  • I confirm that I have disclosed any known allergies or sensitivities

  • I understand that a patch test is recommended but may not always be performed

 

I release Tones Color Lab from liability related to allergic reactions unless caused by gross negligence.

Hair Extensions Consent

I understand that hair extensions (I-Tip, K-Tip, or similar methods):

​

  • Require ongoing maintenance every 4–6 weeks

  • May cause tension or stress on natural hair if not properly maintained

  • Can result in breakage if aftercare instructions are not followed

 

I agree to:

  • Follow all aftercare instructions provided

  • Maintain scheduled maintenance appointments

 

I understand that Tones Color Lab is not responsible for damage caused by improper care or failure to maintain extensions.

Hair Condition & Risk Acknowledgment

I understand that if my hair is compromised, previously damaged, or over-processed, there is an increased risk of:

​

  • Breakage

  • Shedding

  • Chemical damage

 

I accept full responsibility for proceeding with services despite these risks.

No Refund Policy

I understand and agree that:

​

  • All services are non-refundable

  • Adjustments must be requested within 48 hours

  • Adjustments do not include changing my mind about the desired result

Photo & Video Release

I give permission for Tones Color Lab to photograph/video my hair for marketing and social media.

Release of Liability

To the fullest extent permitted by law in the State of Texas, I voluntarily release and hold harmless Tones Color Lab, its owner, and staff from any liability for:

​

  • Hair damage or breakage

  • Allergic reactions

  • Unsatisfactory results

  • Any injury or loss arising from services performed

 

I acknowledge that I am receiving services at my own risk.

Client Signature and or Agreement

I have read, understood, and agree to all terms outlined above.
